How CareData Trak’s e-Screening Tool Works

The Kiosk

Specifically designed for seniors, our lightweight, desktop kiosk allows staff to take it to where it will be most effective and needed. Touch screens deliver large fonts, inviting colors, personalized introduction media and the ability to touch the screen text to activate audio reading of the text for those that may have vision impairments. Hand-crafted solid wood cabinets which house the kiosk can be designed based on facility décor and aesthetic needs.

The Screening Process

Monitored by a healthcare professional, the patient is seated at the kiosk and, once logged on, can begin the survey module. Our clinically-validated survey module leverages decision-tree logic to navigate content and analytically measures the risk for geriatric syndromes, including falls, incontinence, depression and dementia, based on the patient’s responses. Within 20 minutes, the assessment can be completed and a full report will be generated with detailed results including at-risk diagnoses and treatment categories.

Reporting

To ensure the effectiveness of treatment plans, reporting can be customized to view results and data by individual, population, site, region or any other category. Healthcare providers can track at-risk diagnostics over time and apply targeted treatment plans across individuals and populations. Simple reports can be shared with adult children of the elderly in assisted living facilities to enhance communication.
